Occupational Health clearance and declaration forms
We require all students undertaking professional teaching and health and social care courses to complete an occupational health check. These forms are used to ensure that new students meet the required health standards of their course before starting. Completion is a condition of your entry onto the course.
Please be open and honest when completing the Occupational Health screening forms as this process enables the University to make well-informed decisions about your fitness to train on your chosen course and to consider and support any reasonable adjustments that may be necessary to support you throughout your programme, including your placements.
The aim of the Occupational Health Check is to ensure that a future teacher, health care practitioner or social worker has both the physical and mental fitness to perform their duties without putting people at risk. Each case will be looked at individually, on its merits and in the light of the particular circumstances. As a university, we have a duty to ensure that health and safety or child protection requirements are not used to justify discrimination against a disabled student (Equality Act 2010).
